In-Home Services
In-Home Services support vulnerable older adults living in their own homes by helping them strengthen their formal and informal support networks, reduce their social isolation and increase their independence. LBFE staff members provide in-home visits and phone calls, act as advocates, make referrals to other services, and help these older adults solve problems.
In-Home Services staff members may also utilize Visiting Volunteers who can offer friendship to these older adults, help reconnect them to their community, and enhance their personal independence. Visiting Volunteers are closely matched with the people we serve according to interests, personality and geography, and they provide monthly status reports to their staff supervisor.
Visiting Volunteers receive orientation, training, and spend 6 hours per month supporting an older adult.
Client Criteria:
- Age 60 and over
- Live independently or in assisted living in Hennepin or Ramsey County
- Weak informal support network
- Lacks regular contact with family or has none
- Has few friends
- Has limited affiliation with community organizations
- Able to hold a conversation and recall staff/volunteer from visit to visit
- Amenable to services from staff and/or volunteers, including phone screening, in-home assessment, signing program forms, home visits
- Open to establishing new relationships
- Emotionally stable
- Socially appropriate
- Willing to share emergency information
